Sunrise Coordination Specialist Support Coordination services, provides the highest level of support for NDIS participants experiencing complex challenges across areas such as housing, health, mental health, justice involvement, or social and environmental factors. This service is designed for individuals with high and complex needs, ensuring they receive coordinated and responsive support to stabilise their situation, improve outcomes, and build long-term capacity.
A Specialist Support Coordinator works in partnership with participants to help them:
Navigate complex service systems, including NDIS, health, housing, justice, and mental health. Overcome significant barriers to accessing and maintaining supports. Coordinate multiple stakeholders, services, and crisis responses. Develop and implement tailored service plans that reflect the participant’s goals and needs. Reduce risks and stabilise challenging life situations. Strengthen informal and formal support networks
At Interaction, our team of Specialist Support Coordinators hold relevant professional qualifications (e.g. in social work, mental health or allied health) and have extensive experience in supporting people with complex support needs. We work alongside participants to deliver. Comprehensive assessments to identify key challenges and barriers. Collaborative planning that puts participant goals, values, and preferences at the centre. Individualised service plans that focus on stability, safety, and progression.
